Around 1930's, clubs start to established especially in big cities such as Labuanjaya, Kembangan, Kartasura, Junggalu and Balawan. An unofficial tournament between this clubs was held in 1948.

With the huge development of the sports, in 1951 with the initiation of Rahmat Masardi, the then Youth and Sports Minister, and the help of basketball practitioner, Anton Surya and Wirya Latuhena, Persatuan Bola Basket Seluruh Matawis (PBSM, All-Matawis Basketball Association) was establish. The organizations first mission was to unite many clubs that spread around the country and help them to fund the facilities to improve the sports appeal in public's eyes. The efforts was success to bring the sports far into the villages. And it compete with football as the most practice sports.

It was not until 1982 that the association finally launch its first league, an amateur league called Kompetisi Bola Basket Utama (KOBATAMA, Premier Basketball Competition). Before that, there has been numerous competitions held. But most of them were regional based. The first edition of the league was competed by 10 clubs. Most of them were from Jama Island, the most populated island in the country. With only 1 club, Balawan Raya that come from Andalas Island. Labuan Muda was came out as the winner. It become the sole competition in the empire until 2003 when PBSM launched its semi pro league, Matawis Basketball League (MBL). Its operated with a promotion-relegation system with Kobatama. 22 clubs were compete within both leagues, 10 in MBL and 12 in Kobatama. This system however only lasted for 6 years. The 2009 edition become its last edition after many clubs went bankrupt and leave the league.

There were no competition held between 2010-2015 after PBSM propose a franchise tournament system to promote professionalism in the sports. The association got inspiration from the football league who already start its professional league in 2005. The previous 22 clubs were invited to joined. But as the time passed, only 12 of them that ready to the new system. In 2016, the new league was launched and it is call National Basketball League. By thinking about transport and accommodation, the organizer goes with series system where the matches will be divided into several series with different city will host the series. Benteng Labuanjaya came out as the champion in the first season. But it was Satria Junggalu who managed to dominated the league. They collected 3 titles and 4 runner-ups. Starting the 2025 season, the organizer decided to abolished the division system, adopting the same moved made by the football league in 2021.

Season format

In its earlier seasons, the league was divided into 2 divisions, Kulon (West) and Wetan (East) Divisions. Then the matches for the regular season are divided into several series, 4 to 8 series. Then the championship series was competed by top 4 from each division to search for the ultimate champion. The quarterfinal and semifinal was using best-of-5 match while the final using best-of-7 match. Starting from 2025 season, the regular season change as both divisions are fused into a single division league. But the series are still implemented and it is fixed at 8 series per season. The cities selected as the host of the series will be interchange between the former Kulon clubs and Wetan clubs. And they start with Kulon in the 2025 season.
